Fluticasone propionate
Indications/Uses
Prophylaxis & treatment of seasonal allergic rhinitis (including hay fever) & perennial rhinitis.
Dosage/Direction for Use
Adult, elderly & childn ≥12 yr 2 actuations in each nostril once daily to bd. Decrease to 1 actuation in each nostril daily once adequate control is achieved. Max daily dose: 4 actuations in each nostril. Childn 4-11 yr 1-2 actuations in each nostril once daily. Max daily dose: 2 actuations in each nostril.
Contraindications
Special Precautions
Should not be used in patients who have experienced recent nasal septal ulcers, surgery or trauma until healing has occurred. Increased risk of systemic effects w/ larger doses. Possible adrenal insufficiency & w/drawal symptoms in patients transferring from systemic to topical corticosteroid. Increased susceptibility to infection. Use w/ caution in patients w/ systemic infections, active or quiescent TB infection, or ocular herpes simplex. Avoid exposure to chickenpox & measles. Must not be inhaled in acute bronchospasm or status asthmaticus. May cause HPA axis suppression particularly in younger childn, in patients receiving high doses for prolonged periods, when used on large or denuded areas of the body, or when used w/ an occlusive dressing. Avoid spraying to the eyes. Concomitant use w/ other inhaled corticosteroids. Pregnancy. Contraindicated in nursing mothers. Ped patients <3 mth; may cause growth velocity reduction in ped patients.
Adverse Reactions
Headache, fever; nausea/vomiting; muscle injury, musculoskeletal pain, back problems; upper resp tract infection, throat irritation, nasal congestion, pharyngitis, oral candidiasis, sinusitis/sinus infection, rhinitis, dysphonia; viral infection.
Drug Interactions
Increased serum level &/or toxicity w/ ketoconazole (CYP3A4 inhibitor). Potentially increased serum levels w/ amiodarone, cimetidine, clarithromycin, delavirdine, diltiazem, dirithromycin, disulfiram, fluoxetine, fluvoxamine, grapefruit juice, indinavir, itraconazole, nefazodone, nevirapine, propoxyphene, quinupristin-dalfopristin, ritonavir, saquinavir, verapamil, zafirlukast, zileuton. Improved response to inhaled corticosteroids w/ addition of salmeterol. Decreased serum levels w/ St. John's Wort (CYP3A4 inducer).
